Please fill out this form in its entirety. Make sure you have the correct information for all officers and your advisor, information about current and future programs your organization will sponsor for this year, and any awards or special recognition your organization has received for the past academic year.

Please also include your student organization's constitution and bylaws so that we can upload them easily.
